Updates to Loan Service Forms

We have updated a few forms on SharePoint commonly used to make a little more user friendly and universal.

ACH Auto Payment Authorization is now the Loan Account Services Form

     • One autopay form for all loan types and credit cards

     • ODP set up request for HELOCs, Credit Card, and other LOC items

Credit Card Change of Request is now the Credit Card Balance Transfer Form

     • Autopay and ODP have been moved to Loan Services Form, leaving the just the Balance Transfer form if needed.

Please make sure you are using SharePoint instead of any forms saved to your desktop for continuity and updates made.
